Condensation Between Window Panes
Moisture on the room-side surface of glass can be a humidity issue. Moisture or fog trapped between panes is different: it generally indicates the sealed insulated-glass space is no longer intact.

Insulated glass units are factory-sealed. When that seal fails, moisture can enter the space between panes and create haze, droplets or mineral residue.
Glass-only vs full-window replacement
Some window systems allow the insulated glass unit to be replaced while retaining the sash or frame. Full replacement may make more sense when the frame, sash, operation, or other units are also deteriorated.
Do not confuse surface condensation
Interior surface condensation can occur even with a functioning insulated-glass seal when indoor humidity, outdoor temperature, glass temperature and ventilation conditions align.
